Why ibogaine cannot detox benzodiazepines

Benzodiazepines (Xanax, Klonopin, Valium, Ativan and others) work by enhancing GABA, the brain's main inhibitory system. Physical dependence is a GABA-system adaptation, and relieving withdrawal requires acting on that system. Ibogaine essentially does not: its activity is on opioid, NMDA, serotonin and sigma targets, not GABA.^[1] So unlike with opioids, where ibogaine can blunt withdrawal, it does nothing to relieve benzodiazepine withdrawal. Using it for that purpose is trying to fix a lock with the wrong key while the danger clock keeps running.

Why the combination is dangerous

This is not merely ineffective, it is actively risky, for two reasons that stack. First, benzodiazepine withdrawal is itself potentially fatal: unlike opioid withdrawal, unmanaged benzodiazepine withdrawal can cause seizures and delirium. Second, that seizure risk lands on top of ibogaine's cardiac risk, and benzodiazepines have appeared as a contributing factor in reported ibogaine deaths.^[2] The guidelines the field uses are explicit that benzodiazepine dependence must be addressed before ibogaine treatment, not during it.^[3]

The correct sequence

For anyone dependent on benzodiazepines, the safe path is a slow, medically supervised benzodiazepine taper first, completed and stabilized before ibogaine is even considered for any other issue. This is not optional and it is not fast: benzodiazepine tapers are measured in weeks to months. A clinic that offers to "handle your benzos" as part of an ibogaine program, or that dismisses your benzodiazepine use as a non-issue, does not understand the pharmacology or is choosing to ignore it. Either way, walk.
